Mir, Dir German Grammar

Mir geht's sehr gut. (I am very well.)
Mir fehlt ein Buch. (I am missing a book.)
WHy we use mir at first?
Is it inverted?

Der gefa'llt mir!
Der ist aber toll.
Why we use Der for the subject?
How come we use grammatically?
and please intepret those sentences.
